Dear Parent(s) / Guardian(s) of prospective Handi*Campers:

Thank you for sending your children to our Handi*Camp at Mill Stream Bible Retreat Centre and Bible Camp, in Omemee, ON (in the Peterborough area)

• CAMPER STATEMENT FOR ACCEPTANCE TO HANDI*CAMP

• CAMPER APPLICATION FORM – Please read each section carefully as you complete it. We take the health of your children seriously. The information you provide is essential for planning for the overall health of each camper and staff member.

• CAMPER PERSONAL PROFILE FORM -This will be photocopied and given to your child’s Counselor and made available to his/her Bible Teacher and other leadership staff so they will know about your child’s interests, activities and other important information.

• CAMPER HEALTH HISTORY FORM – This form will provide ready access to immunizations, allergies and health history information as well as medications, insurance and other important health areas. Please complete these pages carefully. If you don’t have your child’s current immunization record, your doctor’s office or your child’s school can perhaps make a copy for you. It’s important that these dates be completed. NOTE: Only the most recent immunization date in each series is needed (not each of the DPT shots, for instance). Additional note concerning medications: Please call me at our office, and I will put you in contact with Linda Costley our Handi*Camp Head Nurse, if you have questions about sending medications. Our nurses cannot legally give any medications that are improperly labeled  (including over-the-counter} medications.

• ALSO - Check to be certain you have completed and signed in ink all the release statements on page 3. Complete and return Section 2 with a cheque made out to BCM International (Canada) Inc.designated HANDI*CAMP when you return your application papers. Section 1 is for your records.

• CAMPER PHYSICIAN'S FORM - This form must be signed by a licenced practicing physician.

• CAMPER FEE INFORMATION SHEET - Please carefully review the information regarding camp fees.
